系统阐述 TGHDL-C 在代谢综合征状态下与肾功能损伤的潜在联系

摘要(Abstract):

代谢综合征(MetS)是一种全球高发的疾病,其核心病理包括胰岛素抵抗和致动脉粥样硬化性血脂异常,典型表现为甘油三酯(TG)水平升高与高密度脂蛋白胆固醇(HDL-C)水平降低。近年来,TG/HDL-C比值作为一种能综合反映脂代谢紊乱与胰岛素抵抗的简易指标,在代谢风险评估中日益受到关注。大量研究证实,TG/HDL-C比值的升高与慢性肾脏病(CKD)的发生和进展显著相关。其潜在机制涉及动脉粥样硬化性微血管损伤、脂毒性引发的肾小管上皮细胞损伤、增强的炎症与氧化应激反应,以及由胰岛 素抵抗介导的肾小球高滤过和纤维化进程。因此,TG/HDL-C比值有潜力成为MetS相关肾损伤的早期风险标志物,弥补传统指标(如估算肾小球滤过率eGFR和尿蛋白)在早期识别中的不足。未来的研究方向包括通过多中心前瞻性研究和干预性试验,进一步确立该比值在不同人群中的预测临界值及临床应用价值,并探索其与新型生物标志物和风险模型的整合,以构建更完善的评估体系,最终提升CKD的早期预警与精准防治水平。
