摘要(Abstract)：

目的 探讨经内外侧联合双窗入路治疗复杂距骨骨折的方法和临床疗效。方法 回顾性分析我院自2020年1月2023年1月收治并行切开复位内固定的复杂距骨骨折35例，其中男29例，女6例，距骨颈骨折27例，距骨体骨折8例，观察收集所有患者术后骨折愈合及相关并发症情况，按照踝关节活动度ROM、视觉模拟疼痛评分（visual analogue scale，VAS）和美国足踝外科协会踝-后足评分（American orthopedic foot and ankle society，AOFAS）进行术前、术后3月、术后6月及末次随访临床治疗结果评价。结果 所有患者均获得随访，随访时间19-36个月，平均25.1个月。所有患者骨折均获得骨性愈合，平均愈合时间15.7±4.2周。其中11例出现切口愈合不良，3例患者伤口分泌物培养出细菌，经过支持对症治疗后，切口愈合。末次随访时VAS评分为2.89±0.55分，AOFAS评 分为81.12±14.45分，踝关节ROM为25.52±4.34°，术后整体优良率为82.86%，其中发生创伤性关节炎6例，距骨坏死3例。结论 经过内外联合双窗入路内固定治疗复杂距骨骨折，手术视野暴露充分，有利于骨折的复位，预后良好，有效减少术后创伤性关节炎和距骨缺血性骨坏死的发生率。
