摘要(Abstract)：

目的探讨胆囊十二指肠瘘伴胆石性肠梗阻的诊断难点、一期手术的决策要点。方法报告1例66岁男性患者，因“上腹部疼痛伴恶心、呕吐2天。”入院。CT提示胆石性肠梗阻可能，在多学科协作下，行急诊腹腔镜探查中转开腹，实施小肠切开取石、扭转复位、胆囊切除及十二指肠瘘修补一期手术。结果手术证实为胆囊十二指肠瘘（直径3cm）致回肠末端结石嵌顿伴小肠扭转。术后并发脓毒血症及消化道出血，经针对性治疗后均有效控制。患者术后16天痊愈出院，术后1个月及近期随访均未见并发症。结论对于胆石性肠梗阻，腹部CT是重要诊断手段。即使在高龄、多合并症患者中，若经充分评估手术耐受性尚可，一期根治手术仍是安全可行的选择，但需警惕术后感染与出血风险。
